A macropenis or macrophallus is an unusually large penis. In a medical context, the condition, known as macrophallism, is defined as an adult or juvenile[1] penis with an erect length of 2.5 standard deviations (SD) above the average (approximately seven inches or 17.8 cm in length post-puberty)[2][3] but otherwise formed normally.[4][5][6] It is generally characterized by substantial penile girth or length, sometimes causing functional issues.[4] The condition is occasionally associated with underlying conditions like priapism, palmar fibromatosis, hypertrophy, and sickle cell disease.[5] Treatments for macrophallism primarily include penis reduction.[4]

Terminology

Other terms and synonyms denoting the same condition are megaphallus[7], megapenis[8], macrophallus[9], megalophallus[10] and megalopenis[11]. As an indisposition, disorder or condition, it is sometimes termed macrophallism.[12] When the indisposition is the subject of an atypical sexual interest it is on occasion termed as phallophilia[13], as well as slang terms like size queen.[14] The opposite condition, an unusually small penis, is called micropenis.[15]

Scope

An erect penile length measuring more than 2.5 standard deviations longer than average is considered a macropenis.[16][17][18][19] The average erect penile length is 13.12 (1.66 SD) cm. [20] Hence, macropenis can be defined as a penis whose erect length is at least 17.27 cm.[2][3]

Alternatively defined as penile length more than 2 SD above the mean for age objective or apparently increased penile length for age. In this alternative definition, penile length is the distance between the midline attachment of the gently stretched, flaccid penis above the pubic symphysis and tip of the glans.[21] There may exist a discrepancy for measurement of macrophallism for some growers and showers. In the former (usually among younger men) the penile length may be in the normal range while flaccid but reach macrophallic lengths upon an erection. In the latter a man may have macrophallic penile dimensions while flaccid but is in the normal range while erect. This is due to the variation in tumescing growth of a grower 5.3 cm [SD 0.5] compared to 3.1 cm [SD 0.9] in a shower.[22]

Causes

Macropenis may result from complications from adrenal or pituitary disorders disorders[23], or recurrent episodes of priapism which lead to excessive growth of the penile tissues.[24][25][26] Macropenis in children can derive from multiple causes, including an unusual rise in testosterone production, testicles with interstitial cell tumors, hyperplasia, an adrenal cortex with a tumor, precocious puberty and associated hypothalamic tumors or as a byproduct of other congenital anomalies. It can also occur due to a chromosomal disorder, such as inversion of chromosome 15 or deletion of chromosomal region 13q21q31.[27][28] Some men have acquired the penis size above the median range through deliberate enhancement methods, through the usage of a specific device, a medicinal substance or through manual manipulation such as jelqing.[29]

Diagnosis

Some men with the condition report feeling distressed[30] and may attempt penis reduction surgery.[31] However, a urologist may claim that there are no viable techniques to reduce the penis size,[32] and that macrophallic men should instead be gentle while copulating.[33] Some men on the most extreme size peripheries have claimed to feel light-headed during full erections[34]. Some men only come to the realization of their size anomaly after realizing that standard sized condoms do not fit comfortably,[35] while for others it derives from complaints from a significant other of pain to their cervix, anterior fornix or posterior fornix.[36]

Management
